We have a variety of forms that we will have you fill out depending on student needs upon entering W.A. Day School.

1)  Registration Forms:  These include demographic information (name, address, etc.), FOIPP, Internet Acceptable Usage, and Child Abuse Prevention Program Consent.  We require a Birth Certificate, and Treaty Status Card to register a student at W.A. Day School.

2)  Learning Support Forms:  We work with many outside agencies at W.A. Day, so we must get your permission to share information as well as getting your permission for us to do further assessments.  These forms are filled out on an as need basis.  You may never see these forms at our school.

3)  Providing Medication Forms for students must be filled out by the Doctor before we can administer any medication.  There are special forms for that so please check with us to get that organized if necessary.

4)  We are part of the program for Medic Alert that provides free medic alert bracelets for severe needs.  If you think your child has these kinds of needs, please contact Mrs. Woynarowsky for a form when you register.  We prefer to be safe with the health of our students.
